- Submit a copy of Certificate of Grades from the start of entrance in the University for evaluation purpose to the Registrar's Office
- Accomplish re-admission form.
- Secure medical referral from the Health Service Unit then undergo medical examination
- Submit accomplished re-admission form and medical clearance to he Registrar's Office
