Schedule for Adding, Dropping and Changing of Subjects/Schedules

  • September 25 - October 12, 2023 – Adding, Changing of Subjects
  • September 25 - October 12, 2023 – Dropping of Subjects with Evaluation
  • October 19, 2023 – Last day of Dropping of Subjects without Evaluation


1. Adding, dropping and changing of subject/schedule is NOT ALLOWED for REGULAR STUDENTS.

2. Download and print the form thru this link

Fill out this part of the Form:

Valid reasons for Adding, Dropping and Changing of subjects are the following:

  • subject is not needed 
  • ill-advised 
  • conflict in schedule
  • registered higher subject without passing the prerequisite subject(s) 
  • registered major course without passing all the required basic courses.

Example:


a. For Adding of Subjects the student needs to fill out the Schedule Code, Subject Code and Units of the subject to be added. Write the name of the Instructor and have it signed by the instructor or representative from their department. Example below:


b. For dropping of Subjects the student needs to fill-out the Schedule Code, Subject Code and Units of the subject to be dropped. Write the name of the Instructor and have it signed by the instructor or representative from their department. Example below:


c. For Changing of Schedule the student needs to fill out the Schedule Code, Subject Code and Units of the subject to be changed. Write the name of the Instructor and have it signed by the instructor or representative from their department. Example below:


2. The student will submit the hard copy of Adding/Dropping/Changing Form to their Registration Adviser for evaluation and signature. 

3. Accomplished form will be submitted by the student to the Office of the Campus Registrar for checking and validating the availability of the subject. 

4. After validation by the Registrar, submit the accomplished form to the MIS office for encoding.

NOTE:  Signatures from your instructor, registration adviser and registrar's office must be original and complete in order to process your form.
