REQUIREMENTS

          1. Accomplished application for admission form. Must be A4 size and printed back to back (Click to download)

          2. Photocopy of Report Card

          3. Photocopy of Certificate of Good Moral

          4. Two Copies of 1 x 1 ID picture

          5. Short white folder

            ADMISSION AND ENROLLMENT PROCEDURES

                                    1. Evaluation of School Credentials

                                    2. Entrance Examination

                                    3. Issuance of Notice of Admission (NOA) for examination passer at the Guidance Office

                                    4. Get the medical referral slip at the campus clinic

                                    5. Take the interview  at the Department of your chosen course. Have the NOA signed if you'are accepted to the program

                                    6. If accepted, proceed to the date of medical examination. If not, go to the Guidance office

                                    7. Claim the medical examination result at the campus clinic

                                    8. Submit all the requirements at the Campus Registrar

                                    9. Fill up the  Student Profile form then submit to the Campus Registrar to have your student number

                                    10. Wait for the announcement of claiming your Registration form

                                    11. Wait for the announcement for picture taking for school ID

                                    12. Validation of Registration form and school ID 


                                    REQUIREMENTS

                                    1. Accomplished application for admission form. Must be A4 size and printed back to back (Click to download)

                                    2. Photocopy of Transcript of Records (TOR) or Certificate of Grades (COG) signed by the registrar 

                                    3. Photocopy of Honorable Dismissal or transfer credentials

                                    4. Photocopy of Certificate of Good Moral

                                    5. Photocopy of NBI clearance (18 years old and above) or Police Clearance (17 years old and below)

                                    6. Two Copies of 1 x 1 ID picture

                                    7. Short white folder

                                    If the applicant is from other CvSU branch campuses:

                                    * No need to take the entrance examination, unless special condition

                                    1. Photocopy of TOR or Certification of grades signed by the Registrar

                                    2. Photocopy of Honorable Dismissal

                                    3. Two Copies of 1 x 1 ID picture

                                    4. Short white folder

                                    POLICY:

                                    1. GPA must be  2.00 or better without failing grade

                                    2. No transferee or shiftee shall be admitted for other programs  after the second year level

                                    ADMISSION AND ENROLLMENT PROCEDURES

                                    1. Interview/ Evaluation of School Credentials  at the Guidance Office/ Registrar's Office

                                    2. If passed the evaluation/interview. Student will take the Entrance Examination

                                    3. Issuance of Notice of Admission (NOA) for examination passer at the Guidance Office

                                    4. Medical Examination

                                    5. Proceed to the date of medical examination

                                    6. Claim the medical examination result at the campus clinic

                                    7. Submit all the requirements at the Campus Registrar

                                    8. Fill up the  Student Profile form then submit to the Campus Registrar to have your student number

                                    9. Wait for the the schedule of enrollment for transferees

                                    10. Accomplish the  Pre-registration form then submit to the MIS office

                                    11. Claim your registration form then wait for the schedule of picture taking for school ID

                                    12. Validation of Registration form and school ID

                                    1. Accomplished Re-admission form at Registrar's Office

                                    ADMISSION AND ENROLLMENT PROCEDURES

                                    1. Issuance of Re-admission form

                                    2. Evaluation of subjects/grades

                                    3. Medical Examination

                                    4. Submit all the requirements at the Campus Registrar

                                    5. Wait for the the schedule of enrollment

                                    6. Accomplish the  Pre-registration form then submit to the MIS office

                                    7. Claim your registration form

                                    8. Validation of Registration form and school ID 


                                    REQUIREMENTS

                                    1. Accomplished Letter of Intent at Registrar's Office

                                    POLICY:

                                    1. GPA must be  2.00 or better without failing grade (For those students admitted from Academic Year 2019-2020 onwards)

                                    2. No transferee or shiftee shall be admitted for other programs  after the second year level

                                    3. Students who want to shift to other program should submit  approved Letter of Intent not later than 10 working days during registration period.

                                    ADMISSION AND ENROLLMENT PROCEDURES

                                    1. Issuance of Letter of Intent

                                    2. Evaluation of subjects/grades

                                    3. Accomplish the Equivalency Form

                                    4. Submission of Approved Letter of Intent

                                    5. Submit all the requirements at the Campus Registrar

                                    6. Wait for the the schedule of enrollment

                                    7. Accomplish the  Pre-registration form then submit to the MIS office

                                    8. Claim your registration form

                                    9. Validation of Registration form and school ID
